The following forms are available in packets at Courthouse and some require a fee. You may purchase these forms in packets at the courthouse or you may access them by clicking on the links below. Please note, the forms listed below may not include every form you may need for your case. Filing an action with the Superior Court normally requires a filing fee. If you are low-income, you may be eligible for a fee waiver which would waive the fees for filing an action with the court. Below are links outlining the qualifications for a fee waiver and the forms to request a fee waiver:

Small Claims Court is a quick and inexpensive way to resolve disputes that do not exceed $7,500.

An Unlawful Detainer is an action by which a landlord seeks to evict a person or persons from rented property, either residential or commercial.
Probate/Conservatorship – A probate case is filed when a person’s estate is unresolved. A conservatorship case is filed when an adult person is unable to care for themselves and/or their finances.

A petition for name change may be filed by a person 18 years or older to legally change their name and may also be filed on behalf of persons under 18, such as their child(ren).
